Agencies - Sign up for table at the 2024 Health Fair here

The Annual Community Health Fair/Family Fun Day is scheduled for Saturday, June 01, 2024, at Grace Community Church 525 Glenn Avenue, Washington C.H., Ohio.

Screenings will take place in the church but this year participants have the choice of setting their agency table up IN the church OR outside.  Registration fee for setting up inside will be $35 and outside $25.  Inside, the tables will be placed in entryways, hallways or as space allows leaving most of the auditorium free for the Columbus Zoo presentation.  

Please note tables this year will be 6'and not 8'.

If you choose to set up outside we recommend bringing a pop-up tent.  Hours will be 8 am to 12 noon inside and 9 am to noon, outside.

Set up inside will be on Friday, 5/31/24 from 12:00 noon to 4:00 pm.  We recommend set up outside on Saturday morning between 8:00-9:00 am.  

Please note on the registration form if you will be located inside or outside.  It is our plan to have everyone setting up outside to be closer to the front of the church this year for improved flow and easier access for those attending. Outside agency tables and activities will be canceled if it rains.

Please note on the registration form if you are willing to provide an item or two to be used by the committee to make up three large coolers to be offered as the door prizes.
